This session will cover the work undertaken at Eastern Health, a large multi-site healthcare provider in Victoria, Australia, over the past three years to introduce seven day services across General Medicine. The session will describe the imperative to make the changes, what has been done and what has been achieved in delivering consistent seven day services across Eastern Health.
The genesis of the redesigned model of care came from data that showed variation in length of stay depending on which unit and acute site patients were admitted to resulting in poor patient experience.
The outcomes have been measured across a number of qualitative and quantitative indicators and highly impressive results have been achieved. The session will describe the work from the perspective of the clinician leader, the operational manager and the improvement lead.
